Children's Connection FAQs

How do I qualify for Children’s Connection services? Do I need a referral?

Anyone can call our office directly for services. You can also be referred by your child’s school or your primary care physician. A referral is not needed.

How much will this cost me?

There is no charge for the Children’s Connection services.

What if I live outside of your normal travel area?

We will do our best to accommodate your needs. Office visits are available to anyone.

How can you help me with my child’s asthma?

Through asthma education we can help you to manage your child’s asthma by identifying the following:

  • Personal warning signs
  • Personal triggers
  • Asthma medications
  • Use of a peak flow meter which is used to monitor your child’s asthma

Together we will create an asthma action plan

Will you work with my child to improve his behavior at home?

We talk with you about what has already been tried and what has and hasn’t worked in the past and we work together to develop a new plan to implement. Changing your child’s behavior usually has to start with the parent. This is not to imply that your parenting is wrong but only that you be willing to try something different to get different results. We will help you to determine where the behavior is coming from and assist you in finding successful ways to improve your child’s behavior.

What documents do I need to apply for Healthy Kids?

  • Proof of last four weeks of income
  • Proof of any unearned income for past four weeks (social security/child support)
  • Birth Certificate for each child
  • Immunization records for each child
  • Proof of any current insurance or any insurance over the past 6months (copy of insurance cards and proof of expiration)
  • Proof of any court ordered expenses (garnished wages)
  • Picture ID for anyone 16 or older applying for coverage
  • If pregnant your birth certificate, Picture ID, proof of pregnancy.

How long will it take to have Healthy Kids coverage once I do an application?

Usually it takes about 4 weeks. However if your child is being denied health care and qualifies we can complete a presumptive eligibility application, which means your child can become covered within a day or two.

I don’t want the state involved; do you work with the Division of Children Youth and Families (DCYF)?

We are a non-profit community agency; we are not DCYF state workers. Our involvement is strictly voluntary and our work is based on your needs as a family. However, we are mandated by law to report any signs of child or elder abuse.

I want to apply for housing but I don’t know where to begin is this something you can help with?

Yes, we don’t work for the housing authority, but we do have applications on hand and can assist you with completing them.
