Medication Bridge is a program to help eligible individuals of all ages without insurance, or with limited insurance, to receive needed prescription medications. Every major pharmaceutical company has a patient assistance program that provides prescription medications to eligible patients at no cost or for a small co-payment. Medications available through this program are usually used for long periods of time. Narcotic medication is NOT available through the Medication Bridge Program. SeaCare will work with your provider to apply for these medications.
The Medication Bridge Program will charge an administration fee of $10.00 for each three month renewals of prescriptions.
Your monthly household income must be less than 200% of the Federal Poverty Level
You must be taking a long-term medication
SeaCare is proud to announce a new and innovative program called NHRx Connects, which is the first Charitable Pharmacy in NH. NHRx Connects is a “Re-dispensing Pharmacy”, which means that unused medications are collected from a long term care provider and re-dispensed to people in need.
Each year, huge quantities of unused medication are discarded and destroyed due to changes in dosage, medication or discontinuation of medication. Rather than disposing the medication, the volunteer Pharmacist at NHRx Connects receives the medication from licensed facilities, removes the medication from its tamper proof package, provides a new label and re-dispenses it to people in need at no charge. Funding for the operation of NHRx Connects Pharmacy is provided by a grant from the Foundation for Healthy Communities. For more information, call 603-772-8119.
